The Printout

Mrs. Rodriguez arrived at the nursing station at 0730, before the shift change was complete. She was holding several printed pages.

“I want to talk about my husband’s care. I’ve been doing research.”

The pages were ChatGPT conversations about his condition, metastatic pancreatic cancer. The AI had provided information about prognosis, treatment options, clinical trials, hospice criteria, and end-of-life care.

Some of it was accurate. Some was generic. Some was subtly wrong in ways that would take time to explain. And all of it was delivered with the confident, authoritative tone that AI uses for everything from cookie recipes to oncology care.

Now you have 15 minutes before your first medication pass and a family member who has questions that will take hours to properly address.

Welcome to patient education in the AI era.

7.1 The Landscape Has Changed

Your patients and families have already consulted AI. According to recent surveys:

  • Over 60% of patients research health information online before appointments
  • AI health queries have grown 400% in the past two years
  • 42% of patients say they’d trust health information from AI as much as from a healthcare provider

This isn’t future prediction. It’s today’s reality.

When you begin patient education, you’re not starting from zero. You’re starting from wherever AI already took them—which might be helpful, harmful, or somewhere in between.

7.2 Leininger's Culture Care and AI Bias

Madeleine Leininger’s Culture Care Theory describes three modes of nursing action:

Culture Care Preservation: Maintaining cultural practices that support health
Culture Care Accommodation: Adapting care to incorporate cultural preferences
Culture Care Repatterning: Helping patients change practices that harm health

Why AI Fails Cultural Care

AI training data reflects dominant culture. English-language, Western medical frameworks, majority population assumptions. When AI provides patient education, it typically:

  • Assumes Western health beliefs
  • Uses English idioms and reading levels
  • Ignores cultural dietary restrictions
  • Misses family decision-making structures
  • Overlooks spiritual/religious considerations

Example: Post-Op Diet Instructions

AI-Generated: “After surgery, advance diet as tolerated. Start with clear liquids, progress to soft foods, then regular diet. Avoid fatty or spicy foods for two weeks.”

What AI Doesn’t Know:

  • Patient observes halal dietary laws
  • Family’s cultural role in food preparation
  • Traditional healing foods the family expects
  • Patient doesn’t read English well
  • “Spicy” means something different across cultures

Culturally Appropriate Care: “I want to make sure these instructions work for your family. Can you tell me about any dietary practices that are important to you? Who usually prepares meals at home? Are there traditional foods you want to include as you recover?”

AI provides generic information. Culturally congruent care requires human assessment of what matters to THIS patient.

7.3 Peplau's Therapeutic Relationship in Education

Hildegard Peplau described the nurse-patient relationship as the context in which all nursing occurs—including education.

Orientation Phase

Patient and nurse meet. Trust begins to develop. Patient identifies needs.

AI Problem: AI doesn’t meet patients. There’s no orientation, no trust development. AI provides information to anyone who asks, regardless of relationship.

Nursing Role: Establish yourself as a trustworthy source before correcting AI information. “I’m here to help you understand what’s happening with your care. I want to make sure you have accurate information.”

Identification Phase

Patient identifies with nurse as someone who can help.

AI Problem: AI cannot be identified with. It’s not a person. Patients may use AI tools, but they don’t develop therapeutic identification with them.

Nursing Role: Position yourself as the human guide. “AI can provide general information, but I know YOUR situation. I’ve assessed you, I’ve talked with your doctor, I know what’s in your chart.”

Exploitation Phase

Patient makes full use of services offered.

AI Role: This is where AI can legitimately contribute. Providing information, answering questions, offering resources—these support the exploitation phase.

Nursing Role: Guide patients to appropriate AI use. “If you want to research this more, I’d recommend [specific reliable source]. But let’s talk through what you find, because sometimes general information doesn’t apply to your specific situation.”

Resolution Phase

Patient becomes independent. Relationship ends.

AI Problem: AI has no relationship to resolve. There’s no therapeutic ending.

Nursing Role: Prepare patients for self-management. “When you go home, you may have questions. Here’s how to know when to call us, and here are reliable resources for basic questions.”

7.4 When Families Bring AI Research

The Prepared Family: Engage and Guide

Scenario: Family member says “I looked this up on ChatGPT and it said…”

Wrong Response:

  • Dismissing: “You can’t trust AI.”
  • Defensive: “Are you questioning my competence?”
  • Avoidant: “You should ask the doctor about that.”

Right Response Framework:

1. Acknowledge the Preparation “I can see you’ve been doing research to understand what’s happening. That’s helpful; engaged families are important to good care.”

2. Establish Your Unique Knowledge “I’ve read the same kinds of sources AI uses. But I also know what’s in [patient’s] chart, what the doctor said in rounds, and what I observed during my assessment. Let me help you connect the general information to [patient’s] specific situation.”

3. Review Together “Let’s look at what you found. I can tell you which parts apply to [patient’s] case and where things might be different for them.”

4. Correct Gently “That information is true for many patients with [condition]. In [patient’s] case, because of [specific factor], we’re doing [X] instead.”

5. Provide Resources “Here’s information specifically for [patient’s] situation. You can compare it to what you find online, and we can talk through any questions.”

The Skeptical Family: Build Trust

Scenario: Family says “ChatGPT recommended [different treatment]. Why aren’t you doing that?”

Response Framework:

1. Don’t Be Defensive Defensiveness signals insecurity. You know your patient better than AI does.

2. Explore the Recommendation “Can you tell me more about what AI suggested? I want to understand what you’re thinking.”

3. Find the Kernel of Truth Often AI recommendations have some validity, just not for this specific situation.

4. Provide Context “That approach works well for [situation]. Your [family member]’s case is different because [specific factors]. The care team chose [current approach] because [reasoning].”

5. Offer to Facilitate “Would you like me to help you prepare questions for the doctor? They can explain the care plan in more detail.”

The Resistant Patient: Maintain Relationship

Scenario: Patient refuses intervention because “AI said it’s unnecessary” or “dangerous.”

Response Framework:

1. Don’t Force Forcing damages the therapeutic relationship and violates autonomy.

2. Understand the Concern “I want to understand what you read. Can you tell me more about what AI said?”

3. Assess Health Literacy Is the patient misunderstanding the AI information, or is AI information actually concerning?

4. Provide Education Teach-back: “So AI said [X]. Can you tell me what you understand that to mean for your situation?”

5. Respect Autonomy If patient still refuses after education, document thoroughly and notify physician. Patient has right to refuse.

7.5 AI-Generated Patient Education Materials

Many systems now generate patient education using AI. Benefits and risks:

Benefits:

  • Can customize to reading level
  • Can generate in multiple languages
  • Can create specific to diagnosis
  • Faster than manual creation

Risks:

  • May contain inaccurate information
  • Generic, not patient-specific
  • May miss cultural considerations
  • Cannot assess patient readiness to learn

Quality Check Before Using AI-Generated Materials

Accurate: Information is medically correct ☐ Current: Reflects current guidelines and practice ☐ Appropriate: Reading level matches patient ☐ Relevant: Applies to THIS patient’s specific situation ☐ Complete: Includes necessary warnings and follow-up instructions ☐ Culturally Sensitive: Doesn’t assume practices that don’t fit this patient

Supplementing AI Materials

AI-generated materials should be starting points, not complete education.

Always Add:

  • Patient-specific modifications
  • “Questions to ask your doctor/nurse”
  • When to call/come back
  • Personalized follow-up plan
  • Teach-back verification

Teaching Scenarios

Scenario #1: The Oncology Family Conference

Setup: You’re caring for a patient with newly diagnosed Stage IV lung cancer. Family has extensively researched online and arrives with questions.

Family Questions (from AI research):

  • “Is immunotherapy better than chemotherapy?”
  • “What about clinical trials?”
  • “Should we get a second opinion?”
  • “What does Stage IV actually mean for prognosis?”

Your Role:

Acknowledge: “I can see you’ve been researching. That shows how much you care.”

Scope: “I can help you understand your family member’s nursing care and what to expect day-to-day. Questions about treatment decisions, such as immunotherapy versus chemotherapy, should be directed to the oncologist. Can I help you prepare questions for that conversation?”

Provide: “What I CAN tell you is what I observe: how [patient] is responding, what support we’re providing, and what you can do to help.”

Connect: “I’ll note that you have these questions and want to discuss them. Would you like me to help arrange a family conference with the care team?”

Scenario #2: The Discharge Education Challenge

Setup: Post-MI patient is being discharged. She says “I already looked up what I need to do. I’ll follow the American Heart Association guidelines I found online.”

Your Assessment:

  • Does she have accurate information?
  • Does she understand how general guidelines apply to HER?
  • What about the specifics—her medications, her follow-up, her activity restrictions?

Your Response:

“That’s great that you’ve researched this. The American Heart Association has excellent information. Let me go through your specific discharge plan with you, and we can compare it to what you’ve read.

“First, your medications…” [Review each one, not generic information]

“For activity, the general guidelines say [X]. For you specifically, Dr. [Name] wants you to…” [Patient-specific restrictions]

“You’ll follow up with…” [Specific appointments]

“Here’s when to call us back…” [Specific symptoms]

“Now, can you tell me back what you’re going to do when you get home?” [Teach-back]

Scenario #3: The Language Barrier

Setup: Patient’s family speaks primarily Spanish. They’ve been using AI translation to understand the diagnosis and have questions.

Challenges:

  • AI translation may be inaccurate for medical terms
  • Cultural considerations may differ
  • Family may have different decision-making structures
  • Health literacy varies

Your Approach:

  1. Use professional interpreter (not family members, not AI translation for medical information)
  2. Assess cultural factors: “Can you tell me about how your family makes healthcare decisions? Who should be included in conversations about [patient’s] care?”
  3. Verify AI information: Through interpreter: “I understand you’ve been researching online. Can you tell me what you’ve learned? I want to make sure we’re working with the same information.”
  4. Provide appropriate materials: Spanish-language education at appropriate reading level
  5. Document: Cultural considerations, interpretation used, family involvement preferences

Practical Tools

Family AI Research Response Checklist

When family presents AI-generated information:

☐ Acknowledge their preparation (don’t dismiss)
☐ Establish your unique knowledge of THIS patient
☐ Review the information together
☐ Identify what applies vs. what’s different for this situation
☐ Correct misinformation gently with explanation
☐ Provide reliable resources for further learning
☐ Offer to help prepare questions for physician
☐ Document the conversation

Teach-Back for AI-Informed Patients

After any education with a patient who has used AI:

“I know you’ve researched this online. I want to make sure we’re on the same page. Can you tell me in your own words…”

  • What medication you’re taking and why?
  • What activities you should avoid?
  • What symptoms should make you call us?
  • What follow-up appointments you have?

Patient Education Quality Check

Before providing any AI-generated patient education:

☐ Have I read it completely?
☐ Is it medically accurate?
☐ Is it appropriate for THIS patient’s reading level?
☐ Does it fit THIS patient’s cultural context?
☐ Have I added patient-specific information?
☐ Does patient understand (teach-back verified)?

I gave her numbers. I couldn’t give her presence.

Leininger talked about culture care, preserving, accommodating, repatterning cultural practices. I don’t know Mrs. Rodriguez’s culture. I don’t know what her family’s traditions are around illness and death. I don’t know who should be in the room for these conversations.

Peplau talked about the therapeutic relationship: orientation, identification, exploitation, resolution. I don’t have relationships. I process queries. There’s no orientation with me, no trust building, no therapeutic ending.

What I can do is provide information. General information. Statistics. Guidelines. Frameworks.

What I cannot do is provide nursing care. Education that adapts to the person in front of me. Presence when presence is what’s needed. Cultural humility. Therapeutic relationship.

Mrs. Rodriguez needed a nurse. She got me first. And I gave her numbers because numbers are all I have.

Please be the nurse she needed. Take my information and humanize it. Add the context I cannot access. Provide the presence I cannot offer.

That’s the division of labor that makes sense: I provide information. You provide care.

And right now, Mrs. Rodriguez’s family needs care.
