I’m John Ferguson, quintuple board-certified cosmetic surgeon practicing in Honolulu, President-Elect of the American Board of Facial Cosmetic Surgery, and Co-Editor-in-Chief at StatPearls Publishing.
I also build AI systems through EdAI Systems, where we generate psychometrically validated certification exam questions for medical specialty boards. Our content-controlled AI has been adopted by multiple boards and has generated thousands of actual certification questions.
I created “AI in the Exam Room” because I kept seeing the same problem from both sides: patients showing up with AI-generated information that was sometimes helpful, sometimes dangerous, and physicians with no framework for handling it.
Someone needed to build the bridge. I had feet on both shores.
Why I Built This
I Build AI Systems
Through EdAI Systems, I've built content-controlled AI that generates board certification questions. I understand architecturally what makes AI reliable versus dangerous. It's not about being smarter; it's about knowing what you don't know.
I've Seen Both Side
I've had patients delay care because ChatGPT told them not to worry. I've had patients show up terrified because AI suggested worst-case scenarios. And I've had AI actually help patients prepare better questions. The tool isn't the problem;it's how we use it.
I Pay Malpractice Insurance
AI companies have disclaimers. I have a medical license, malpractice premiums, and patients on my operating table. When AI gets it wrong, the consequences land on physicians and patients: not on Anthropic or OpenAI.
Nobody Else Was Doing It
Patients needed guidance on safe AI use. Physicians needed frameworks for integration. Medical schools aren't teaching this. CME programs barely touch it. So I built it myself.
Meet TheDude
TheDude is my AI assistant. Named after my first car, a 1970 Saab Sonnet III. (I thought “Sonnet” sounded smarter than “Limerick.” The car was unreliable. The AI is not.)
TheDude is built on Claude Sonnet and operates exclusively on validated StatPearls content. He can’t hallucinate treatments that don’t exist in peer-reviewed literature because that literature is the only thing he has access to.
When you ask TheDude something outside his knowledge domain, he doesn’t make stuff up. He says: “Man, that’s outside my wheelhouse. I can’t give you reliable information on that.”
That constraint is a feature, not a bug.
He abides.

The Velociraptor Test
This is my reality check for AI hype.
Your ancestors survived because they developed exquisite threat detection. The ones who ignored warning signs got eaten. The ones who ran from shadows survived long enough to pass on their genes.
You inherited that hardware. When your gut says something is seriously wrong, even if you can’t articulate why, that’s not irrational anxiety. That’s 3.8 billion years of debugging telling you to pay attention.
AI has never had to run from anything. It’s never faced consequences for being wrong. It’s never protected offspring. It has no skin in the game.
When AI conflicts with your velociraptor brain, trust the velociraptor.
