If you accept the auto-populated text, your documentation now contradicts what you observed. In litigation, that documentation is evidence against your professional judgment.

Scenario 2: The Generic Care Plan

System generates care plan for “CHF exacerbation” with standard interventions.

Your patient: Also has dementia, lives alone, has no transportation, and refuses to take “water pills” because of incontinence.

The AI-generated care plan addresses the diagnosis. It doesn’t address the patient.

Scenario 3: The Template Discharge Instructions

System produces standard post-MI discharge instructions.

Your patient: Reads at 4th-grade level, has visual impairment, lives in food desert, and just told you they can’t afford the new medications.

The instructions are medically accurate and completely useless.

Rule 1: Never sign documentation you haven’t fully read and verified

  • Routine pattern matching? Probably reliable.
  • Complex clinical judgment? Requires my input.

4. What happens if CDSS is wrong?

  • Low-stakes, easily corrected? May proceed with CDSS input.
  • High-stakes, difficult to reverse? Requires independent verification.

3.5 Smart Pumps: The Override Decision

Smart infusion pumps are one of nursing’s most common AI interfaces. They include drug libraries with dose limits, infusion rate guardrails, and concentration verification.

Hard Stops vs. Soft Stops

Hard Stops: Cannot be overridden. Pump will not proceed.
