Three ways to add the AI question to intake, so it is normalized and documented before the provider walks in. Use whichever fits your workflow.

Version 1: addition to a paper intake form

Health information research

Many patients research their symptoms before a visit. This helps us provide better care.

Version 2: rooming script card (print and laminate)

Ask every patient during rooming

"Before the doctor comes in, a quick question: did you look up your symptoms online or ask an AI like ChatGPT about them? A lot of people do, and the doctor likes to know what you found so they can build on it."

Document: if yes, "Patient reports AI consultation prior to visit"; if no, "Patient denies AI consultation."

Flag for provider if the patient
Stopped or changed a medication based on AI; delayed coming in because AI said to wait; is very anxious about what AI said; or was told by AI to go to the ER.

Version 3: digital / portal check-in language

Q1 (required): Before today's visit, did you research your symptoms using online resources or AI tools (ChatGPT, Google, WebMD, symptom-checker apps)? Yes / No

Q2 (if yes): Which tools? ChatGPT or similar · Google Search · WebMD/Mayo or similar · symptom-checker app · other.

Q3 (if yes, optional): Would you like to share what you learned with your provider? [text field, 500 characters]

Q4 (if yes, flags to provider): Did any of these happen based on what you learned? I stopped or changed a medication · I delayed care because I was told to wait · I was told to go to the ER · none of the above.

Routing
Any selection in Q4 except "none of the above" should trigger a flag in the provider's pre-visit summary.
